ED medications explained: how PDE5 inhibitors work

How do PDE5 inhibitors work?

How effective are ED medications in clinical trials?

“Does it work?” is best answered not by any single anecdote but by pooled trial evidence — and that evidence is best read as population averages, not a personal guarantee. The American Urological Association names oral PDE5 inhibitors as a first-line option that men with ED should be informed about unless contraindicated, and grades that as a strong recommendation [13]. It also estimates that up to 30 million men in the US and 150 million worldwide are affected by ED, which is why the evidence base is so large [13].

The most comprehensive comparison is a network meta-analysis of 118 randomised trials covering 31,195 men. It found every PDE5 inhibitor superior to placebo, with tadalafil and vardenafil ranked most effective; tadalafil edged vardenafil on the IIEF erectile-function domain (mean difference 1.49; 95% CI 0.50–2.50), and avanafil scored lower than tadalafil and vardenafil on one global assessment question [5]. When two of the molecules are compared head-to-head, the efficacy gap narrows further. The nitrate contraindication — a genuine red line

The single most important safety fact about this whole class is the interaction with nitrates. Every one of the four labels lists organic nitrates and nitric-oxide donors as an absolute contraindication [1][2][3][4]. The reason is mechanistic: nitrates also work through the nitric-oxide/cGMP pathway, so stacking them on top of a PDE5 inhibitor can cause an excessive, dangerous fall in blood pressure [7].

Timing matters too. Why these are prescription-only in India

Given how strict the nitrate interaction is, it is no accident that ED meds are not sold over the counter. In India, sildenafil is a Schedule H drug under the Drugs and Cosmetics Rules, 1945. Schedule H medicines cannot be purchased over the counter without the prescription of a qualified, registered medical practitioner [11]. The schedule is revised on the advice of the Drugs Technical Advisory Board within the Central Drugs Standard Control Organization (CDSCO), under the Ministry of Health and Family Welfare [11].

This is not a new or fringe category. Interpol's Operation Pangea XVI in October 2023, spanning 89 countries, found that erectile-dysfunction medicines were again the single most-seized drug class, making up 22% of all seized products (they had been 40% in the prior 2022 operation) [12].
